This species has a gray back with a pale blue to white stripe and a wider bright stripe below. It has a pair of blue bands between their eyes, a darkish spot behind their upper rear eye, and a narrow orange line on their cheek. Lives solitary or forms groups in reef areas, in 1-35m depth.
